Web Development: Purchase Request Form (Rio Rancho) - Outsource.com (Rio Rancho, NM) in Albuquerque, New Mexico For Sale

Need interactive form that customers may fill in online to order items. When the form is submitted, I would like a notice to be sent to me, a purchase order to be sent to the manufacturer, and a order confirmation to be sent to the customer.
I would like to have drop down menus and radio buttons as much as possible.
I only have 4 customers, with various points of contact; 33 products; and 2 manufacturers possible.
As part of the form, it must be required that the customer ackowledge they have reviewed the spec sheet for each item ordered. There should be a link from each item to a spec sheet.
Each customer would select from a uniques list of products (they would select fro 8-10 items, rather than all 33)
